Financial Support Terms and Conditions
16 Days is committed to providing financial support to families with babies in the Neonatal Intensive Care Unit (NICU) through our financial assistance program. By applying for and receiving financial support from 16 Days, you agree to the following terms and conditions.
Eligibility
Be a parent of legal guardian of a baby(ies) currently in a Level 3 or 4 NICU.
Demonstrate financial need in the grant application.
Provide accurate and complete information in the grant application.
Agree to follow-up with 16 Days regarding grant use and impact, including regular check-ins.
Agree that first names and parts of the regular check-ins may be used for publiclicy purposes. When 16 Days shares parts of your story with potential donors, you are paying the gift of time forward to future NICU parents. Non-identifying information will be used.
Selection Criteria and Approval Process
Step 1: Submit your online application. 16 Days will contact you within 24-48 hours to set up a phone conversation.
Step 2: During a phone conversation with 16 Days, we will ask questions to more fully understand your situation. Questions will include background on employment status (including any paid/unpaid leave, disability, unemployment), pregnancy-related questions, family questions, and hurdles to being present in the NICU. We will also discuss program guidelines and expectations: minimum of 4 hours per day, 6 days/week visiting your child in the NICU for full impact of financial support.
Step 3: 16 Days will review your answers and contact you within 24-48 hours with approval status.
Grant Use
Funds are designated to help cover personal expenses that arise directly from having a baby admitted to the NICU.
Funds are dispersed while a baby is in the NICU. If there is an early discharge from the NICU, grant funds will end because we are stewards of donor contributions for current NICU parents.
Misuse of funds may result in the requirement to return the grant and disqualify you from future financial support from 16 Days.
Payment and Follow-Up
During the financial support application, you will indicate the preferred method of payment processing. Options include either PayPal or a check request sent to your home address (allow 7 days processing time).
Recipients are required to respond to regular check-ins within 3 days to confirm that 16 Days funds are being used in accordance to the original application request. Continuation of grant eligibility is contingent upon check-ins.
Complete a post-survey after the completion of your 16 Days financial assistance agreement.
Accuracy of Information
Applicants must provide truthful, accurate, and up-to-date information.
16 Days reserves the right to verify any information provided and request additional documentation.
Non-Transferability
Grants are non-transferable and may not be sold, exchanged, or assigned to another party.
Confidentiality and Privacy
Personal information shared with 16 Days will be kept confidential and used solely for the purpose of processing the grant application and follow-ups.
Aggregated, non-identifiable data may be used for reporting and fundraising/marketing purposes.
Parent(s) and Baby’s(ies’) first name will be shared for marketing purposes.
If you wish to receive SMS/Texts, data rates apply and message frequency will vary.
A Media Release will be signed as well.
Limitation of Liability
16 Days is not responsible for any delays, errors, or miscommunications that may occur in processing applications or payments.
The organization is not liable for any consequences resulting from the use of funds.
Agreement
By applying for, and accepting, financial support from 16 Days, Inc., you acknowledge that you have read, understood, and agree to these terms and conditions. You accept responsibility for your financial support request to 16 Days, Inc. and do not hold 16 Days accountable for results that may occur because of receiving financial support in the form of a 16 Days Grant.
