I hereby grant permission to 16 Days, Inc. and its employees, agents and assigns, the right to use, reproduce, transmit and publish, in whole or in part, individually or in conjunction with other images and footage:

1. the photograph(s) I submit to 16 Days;

2. the statement(s) or testimonial(s) that I submit to 16 Days, including information submitted via weekly check-ins; and/or

3. the video recording(s) or audio recording(s) submitted by me to 16 Days, including the transcript of any recording(s)

for media purposes including promotional presentations, media, news press releases, online/internet videos, editorial, marketing campaigns or any other purpose, and in any medium now known or later developed without payment or any other consideration.

I agree that 16 Days, Inc. will be the owner of images, footage, transcripts, and recordings in which my baby(ies) or I appear, including copyright interests. I grant 16 Days, Inc. the irrevocable right, title, and interest to use the name, likeness, image, voice, appearance, recording(s), transcript(s), and performance of myself and my baby(ies). This grant includes, without limitation, the right to edit, digitally enhance or alter, publish, distribute, mix or duplicate and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalities of these images or recordings or other compensation arising or unrelated to the use of my image or recording. I also understand that this material may be used in diverse settings and within unrestricted geographical area.

I grant 16 Days, Inc. the right to use and distribute images, footage, recordings, testimonials, either in whole or in part, for any purpose that 16 Days, Inc., in its sole discretion, may determine necessary, including without limitation to social media and marketing purposes. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.

I agree that I have completely read and understand the above release and agree to release 16 Days, Inc. and its employees, agents and assigns from any claims, actions and liability in connection with such use of any photograph(s), statement(s), testimonial(s), or video/audio recording(s) made by 16 Days, Inc.

I understand that in accepting this financial assistance, all of the information above will be used. In order to have pseudonym in place of actual names, it is my responsibility to contact Julia Seper via email at jseper@16days.org the same day that I submit photos, statements, testimononials, and recordings to 16 Days.
